@Thomoori @nomadatlatl Thanks. I'm confused, though.
I thought "この漢字の読みは何だっけ" meant "what's the reading of this kanji again??" As in, you used to know the reading, but you can't quite remember. It's on the tip of your tongue.
Doesn't "この漢字の読みは何ですか" just mean "What is the reading of this kanji?"

@yuen02 Generally speaking, I think 何だっけ refers to 何ですか, while 何でしたっけ refers to 何でしたか in the formal form. So I guessed you could say この漢字の読みは何ですか instead as in trying to remember how to read this kanji. (This is my personal impression)

この漢字の読みは何だっけ sounds pretty casual, so this phrase isn't suitable for except people in a close relationship like family members or intimate friends.
Aside from it, what I can think of are . . .
1) この漢字の読みは何でしたっけ → this sounds a bit less casual, so I think you could use it more widely.
2) この漢字の読みは何でしたか？ → this sounds polite, so you might be able to use it much more widely. That being said, this somehow sounds slightly weird to me.
3) この漢字はどう読めばよかったですか？ → this sounds pretty formal.
As formal alternatives of この漢字の読みは何だっけ, I would use 1)この漢字の読みは何でしたっけ in daily conversations to make myself sound less casual. And if I need to be more formal, I'd say 3) この漢字はどう読めばよかったですか.
