A concussion is a head injury. Can you clarify a little bit? If I understand correctly, you can say, "I want to apologize on behave of my friend".

sorry, I made my sentence confussing. so this is a scene. I saw a girl and his back seems my a friend. I said “Hi！”. when she turn around I found she is not. so how do I explain the reason.

You can say, "I'm sorry, I mistook you for my friend!". You can also replace mistook with confused if that's easier for you.
